Initial Intake Visit (75-90 minutes) – $600 $500

Included in this fee is:

    • 90+ minutes reviewing intake forms and records
    • 75-90 minute visit with the provider
    • 30+ minutes finalizing your individualized recommendations (available within 72 hours after your visit)

During your initial visit, a thorough review of your intake questionnaire will be done, and personalized testing is generally recommended depending on your health goals. Testing costs are not included in the appointment cost.

Follow-Up Visit (45 minutes) – $300 $225

(Each Additional 15 minutes – $75)

Included in this fee is:

    • 45+ minutes preparing for your visit
    • 45-minute visit with the provider
    • 30+ minutes finalizing your recommendations (available within 72 hours after your visit)

Comprehensive Lab Review Visit – for existing members (60 minutes)- $450 $375

Included in this fee is:

    • 60+ minutes reviewing your labs and preparing for your visit
    • 60-minute visit with the provider
    • 30+ minutes finalizing your recommendations (available within 72 hours after your visit)

Cancellation Policy: Embodied Integral Wellness requires 72 business hours (Monday- Thursday) notification via the online Charm portal messaging system for cancellation or to reschedule. However, if you have a Monday appointment scheduled, you need to cancel by Thursday of the previous week.

-$275 cancellation fee for Initial Intake

Judi Epstein, MSN, ARNP and Embodied Integral Wellness (EIW) is not contracted with any insurance companies and does not accept health insurance and is also opted out of Medicare. Clients will be charged via a credit card, debit card, FSA card, or HSA card on file when you schedule your visits. Upon request, we can provide you with a Superbill which you can submit to your insurance company for reimbursement, as an out-of-network Provider. It is up to you to communicate with your insurance company prior to scheduling your appointment if you have any questions regarding eligibility for reimbursement.
